Attached is the flyer for the PVHS Football Physicals fundraiser. The date is 9/28 at 3:00 pm at BCO in Torrance. See flyer for details. Please wear a mask.
This is a fundraiser for our football program, so all proceeds will benefit PVHS Football.
This fundraiser is for all athletes, so feel free to pass on the flyer.
No need to RSVP.
Also attached are the physical form and a medical history form. Your player should bring these forms filled out along with their payment of $35. (Cash, exact amount, or checks are accepted, made out to PVHS ASB, write football on memo line)
All players must have a physical on file prior to in person workouts, which could be starting at the beginning of October according to our district superintendent. The exact date will be made public as soon as we know.
